Log in to mySFA and select the “Update Address and Phone” link under My Contact Info on your homepage.
Former students
Complete the address change form (PDF) and submit it to the Office of the Registrar in person or by mail using the contact information at the bottom of this page.
Date of birth correction
Supporting documentation required
- Valid driver's license or passport.
Current students
Complete the online request to change student information.
Legal name change
Supporting documentation required:
- Photocopy of a valid driver's license and Social Security card.
- May substitute passport, court order or state ID card for one of the two documents.
- For non-U.S. citizens: Current valid passport or official proof of identity, certified by U.S. embassy abroad or by the appropriate foreign embassy in the U.S.
All legal names on the provided documentation must match.

Gender marker update
Supporting documentation required
- official State of Texas document, such as a valid driver's license
- Also may use a valid state ID card, birth certificate or passport
A valid photo ID is required if submitting the form in person.

Social Security number update

- Copy of your Social Security card.
