You may complete intake forms on your patient portal before your scheduled appointment. The following forms are available below if you're unable to access the patient portal.

We recommend that you arrive 15 minutes early to take care of registration and paperwork. Remember to bring your student ID and current health insurance card.

Medical records from SFA

Requesting Medical Records or Immunizations

Remember your ID and insurance card!

You’ll need to have your valid SFA ID and current health insurance card with you every time you visit the health clinic.

Sick Notes

If you're seen by our provider, you may request a sick note at the time of your visit. The note will include the date of your visit and your return-to-class date.

It is important to remember that Health Services sick notes:

must be requested at the time of your visit.
aren't available for prior dates or if you weren't seen by the Health Services provider.
don't guarantee an excused absence.

We encourage you to familiarize yourself with HOP policy 02-102.pdf (class attendance) and notify your instructors of your absence.
